Registration as a Youth Singer for the 2025 Tune-Up is FREE! Registration includes music packets and a box lunch*.

A Media Release and Permission statement, agreed to by a parent or legal guardian, is required for registration.

*Those with dietary restrictions should plan on bringing their own lunch.

PERMISSION TO PARTICIPATE

I grant permission for my child to participate in the 2025 National Lutheran Choir Tune-Up. I agree to release and hold harmless the National Lutheran Choir, their agents, officers, employees and volunteers from any and all liability, claims, suits, demands, judgments, costs, interest, and expense (including attorneys' fees and costs) arising from such activities, including any accident or injury to the youth singer and costs of medical services. In the event of an injury requiring medical attention, I hereby grant permission to the staff of the National Lutheran Choir to attend to my child and give necessary medical treatment. In addition, I hereby give my permission to the staff of the National Lutheran Choir (including volunteers) to seek emergency treatment for my child if an accident or serious illness occurs.

MEDIA RELEASE

I hereby grant the National Lutheran Choir (NLC), permission to use or reproduce my child's name, creative work, voice, conversation, photograph and/or likeness (the "recorded content") in all forms of media in any and all of the NLC's press and marketing publications and materials, including website entries, promotional video, and business related presentations, with payment or any other consideration.

I understand and agree that "the recorded content" will become the property of NLC and will not be returned. I hereby irrevocably authorize NLC to edit, alter, copy, exhibit, publish or distribute the recorded content and any publications for purposes of publicizing the work and/or programs funded by NLC for any other lawful purpose. In addition I waive the right to inspect or approve the finished product in which the recorded content appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the recorded content. I hereby hold harmless and release and forever discharge NLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I have read this release before signing below and fully understand the contents, meaning, and impact of this release. I also certify that I am the parent or guardian of the youth singer named on this registration, and do hereby give my consent without reservation to the foregoing on behalf of this person.
